Objective: The aim of this study was to examine in what percent of patients HPV DNA is still persistently
detected in the cervix after cold knife conization for CIN 2/3.
Methods: A retrospective review was performed on 108 women who underwent cold knife conization due
to CIN 2/3 at the University Clinic of Obstetrics & Gynecology, Medical Faculty, Ss. Cyril and Methodius University,
Skopje between June 2005 and June 2006. The samples for pre-conization HPV test were obtained at least
within 3 weeks before conization. All patients underwent HPV testing and cytology between 6-12 months after
conization. The polymerase chain reaction for detecting HPV DNA was performed using fresh cell samples from
the cervix in every women.
Results: In 86 of 108 patients (80.3%), HPV DNAs were detected in their primary cervical lesions prior to conization.
All of the 108 patients have been followed within 6-12 months after conization. After treatment, HPV DNA
was persistently detected in 17(19,7%) but negative in 69 (80,2%) of 86 HPV DNA-positive patients. HPV DNA was
not detected in both HPV DNA-negative patients.
Conclusions: Conization effectively removes HPV infection. Patients with persistent HPV infection after conization should be especially followed because they are at increased risk of developing disease recurrence. HPV
testing is the most sensitive mean of identifying persistence or recurrence early and is therefore capable of optimizing follow-up after the treatment of high-grade CIN.
