Abstract

The rapid digital transformation of the healthcare sector has opened new avenues for patient-centred care. This paper proposes a data-driven healthcare framework using Personal Health Records (PHRs), collaborative filtering, and
predictive modelling. By transforming diverse patient data into structured vectors and calculating similarities, the system enables personalised recommendations, early disease detection, and risk assessment, contributing to more efficient, proactive, and patient-centred healthcare delivery.
